NOTE: If you have already received your 1st treatment by me and you are booking your next appointment you can skip step 2. If it is your first time having the treatment done, then please complete all 3 steps.

After you have booked your appointment, fill out the "Client Intake Form" by clicking the button below. Save and email the completed form to brunettevetstudio@gmail.com.
Please text a clear, well-lit photo, outside, in natural lighting, in daylight, of both brows to 951-760-8351.
Note: If you are unable to electronically fill this form out prior to your appointment, do not stress, you can also fill it out at your appointment. Either way, please still submit a photo of your brows to me via text. If you were able to complete this form electronically, you do not need to sign or print it as I will print and have you sign the day of your appointment to ensure no information has changed.
